Evaluation Forms for Helping Today Grants
The Foundation requires an evaluation report to be completed after a grant has been awarded. All Helping today grants use the same evaluation form.
It is generally expected that projects be completed within the timelines described below. You may complete and submit the form by its deadline or sooner if the project is completed earlier. Contact your program officer at (814) 454-0843 if you have problems meeting this deadline.

Evaluation forms for Urgent Fund Requests are due 30 days after award date.

All Evaluation forms can be mailed or hand delivered to The Erie Community Foundation or e-mailed to David González.
Please note that failure to complete this document may affect future funding.
